• Cashless Treatment: Access to cashless hospitalization services at over 28,000 empaneled hospitals across India.
• Comprehensive Coverage: Covers pre-hospitalization expenses (up to 3 days), hospitalization expenses, and post-hospitalization expenses (up to 15 days), including diagnostics and medicines.
• Family Floater: The ₹5 lakh cover is on a family floater basis, meaning it can be utilized by any member(s) of the family. There is no cap on family size or age.

✔️ Eligibility for Ayushman Bharat

Ayushman Bharat is an entitlement-based scheme. Beneficiary families are identified based on the deprivation and occupational criteria of the Socio-Economic Caste Census (SECC) 2011 for rural and urban areas. Some of the categories include:

• Rural families with only one room, kachcha walls, and kachcha roof.
• Households with no adult member between 16 to 59 years.
• Households with at least one physically challenged member and no able-bodied adult member.
• SC/ST households.
• Landless households deriving a major part of their income from manual casual labour.
• Manual scavenger families, primitive tribal groups, legally released bonded labourers.
• For urban areas, specific occupational categories like rag pickers, domestic workers, street vendors, construction workers, etc.
• All senior citizens (70+ years) irrespective of income (recent expansion).

Note: There is no registration process for PM-JAY. If your family is identified in the SECC 2011 database or falls under the expanded categories, you are automatically eligible.

📝 How to Get Your Ayushman Card (Application Process)

Since Ayushman Bharat is entitlement-based, there is no direct online application form for all. However, you can check your eligibility and get your Ayushman Card (Golden Card) issued through the following methods:

1. Step 1: Check Your Eligibility:
   • Visit the official PM-JAY "Am I Eligible" portal: mera.pmjay.gov.in
   • Enter your mobile number and the CAPTCHA code. An OTP will be sent to your mobile.
   • Enter the OTP and select your state.
   • Search by your name, Aadhaar number, Ration Card number, or HHD (Household ID) number.
   • The system will indicate if you are eligible.
2. Step 2: Visit a Common Service Center (CSC) or Empaneled Hospital:
   • If you are eligible, you can visit your nearest Common Service Center (CSC) or any Ayushman Mitra desk at an empaneled hospital.
   • Provide your Aadhaar card and other required documents (see list below) for verification.
   • The Ayushman Mitra or CSC operator will verify your details using your Aadhaar eKYC (fingerprint or OTP).
   • Once verified, your Ayushman Card (Golden Card) will be generated. You can get a printout immediately or download it later.

🔍 How to Check Ayushman Card Status

You can check the status of your Ayushman Card application or the validity of your existing card online:

1. Visit the Beneficiary Portal: Go to the official Beneficiary Identification System (BIS) portal: beneficiary.nha.gov.in.
2. Login: Select "Beneficiary" and enter your mobile number. Click "Verify" to receive an OTP. Enter the OTP and CAPTCHA to log in.
3. Search for Details: Select your State and District. Then choose the scheme (PMJAY) and search by Aadhaar Number, Family ID, or Name.
4. View Status: The portal will display your Ayushman Card status (e.g., Approved, Pending, Rejected) and details of family members covered.

Once your Ayushman Card is generated/approved, you can download it using various methods:

• Through Beneficiary Portal (beneficiary.nha.gov.in):
  • Log in using your registered mobile number and OTP.
  • Search for your beneficiary details.
  • Authenticate with Aadhaar OTP.
  • Click on the "Download Card" option to get it in PDF format.
• Through Ayushman App:
  • Download the official "Ayushman App" from Google Play Store.
  • Log in with your registered mobile number.
  • Navigate to the Ayushman Card section, verify your identity, and download.
• Through DigiLocker:
  • Log in to your DigiLocker account (or create one).
